Year 8 HPV Vaccination Consent Letter

HPV vaccinations will be taking place for Year 8 students in College on 5th June. Further information with the link to the consent form can be found in the attached letter here.

It is really important that you complete the E-Consent form. Even if you decline the vaccine, Vaccination U.K. still need the consent form completed. This will ensure that your child does NOT get vaccinated.

Please be aware that the deadline for the completion of this E-Consent is 24th May.
